The Circle of Honour Awards is about First Nations honouring First Nation's people who have demonstrated tremendous achievements, discipline, vision, and excellence. Nominations are being accepted in the areas of Education and Training; Sports, Culture, Youth & Recreation; Health & Social Development; Economic & Community Development; Lands & Resources; Justice and Indian Government, SFN Veterans Association and Women’s Council.

Eligible Candidates
Eligible individuals or groups must belong to a Saskatchewan First Nation living on or off reserve. Ensure to include all pertinent information on the candidate, as all contact information will be recorded in a database.

Posthumous Award
Is an individual who has gone on to the spirit world that has demonstrated tremendous achievement, discipline, vision and excellence during their lifetime. Ensure to include all pertinent information on the candidate, as all contact information will be recorded in a database. Be sure to include contact information on the next of kin.

Nomination Category
Please check the appropriate box to identify the category (s) that best represents the candidate you are nominating. A candidate can be eligible for more then one category.

Reason for Nominating Candidate
Provide a statement regarding the intellectual qualities and potential of the candidate. If acquainted with the candidate though non-academic interests, please give detail on the character of the candidate.

List of Reference
Submit two (2) references. Ensure that the names, addresses and contact numbers of your references are recorded accurately and legibly in the space provided. Your reference will assist the selection committee in choosing the most qualified candidate for the award. The individual providing the letter of reference cannot be related to the candidate.

Supporting Documents
If the candidate’s category is educational in nature, please provide an official transcript of the candidate’s marks from the current or previous year or any other supporting documents.

If the candidate's category is of a business nature, any copies of financial statements provided will be strictly confidential and will be disposed of immediately after the selection.

Provide details of achievements (i.e. type, date, year, etc.) that may include the following examples:

  • Athletic Achievements
  • Awards, Prizes
  • Extracurricular Activities
  • Volunteer Activities
  • Offices Held
  • Community Involvement
  • Financial Management Training
  • Personal Development and Diversity

Signatures
Application must be signed, the nominator acknowledges and verifies that the candidate is eligible for the specified nominations, as outlined in the criteria.

The candidate must sign and accept the nomination and agrees that the Saskatchewan First Nations Circle of Honour committee may use his/her information to promote the awards and must release his/her name to the public and media.

Under Age of 18
If the individual is under the age of 18, a parent or guardian may sign on his/her behalf.

Deadline Date
All nominations must be complete, including two letters of reference, resume or biographical equivalent of the candidate and must be received and postmarked by April 28, 2007.
